User Ready Forms

User ready forms are required FITBIR specific forms that are heavily used by researchers for data submission. Some of the FITBIR specific forms include Demographics, Medical History, Injury History, and many others. These forms are designed to be diverse enough to capture all the needs of researchers who are collecting various types of data within certain categories.

These forms are available to the FITBIR community in a few different formats:

1.A Word document that represents the case report forms that can be used immediately in a study.

2.A data element report in CSV format that provides detailed information about each "question" in the form.

3.A REDCap Data Element report guide which is in 80% alignment with the REDCap standard. Please note that further refinement of the file is needed in order to upload correctly into the REDCap system.

Access the type of form from the accordion and you will be taken to the Data Dictionary where you can find the word document under "Documentation". The Excel CSV and the REDCap CSV can be found in the top right pane of the Data Dictionary page as well.
